Nausea hit 29% of people on the 10 mg dose in Zepbound's weight-loss trials, against 8% on placebo. The bigger story is when it happened: Eli Lilly's label says most nausea, vomiting and diarrhea showed up during dose escalation and faded over time. That timing explains a lot about Mounjaro side effects, and it's the part most people never get told.
Mounjaro and Zepbound are the same molecule, tirzepatide, sold under two names for type 2 diabetes and weight management. Below are the actual rates from both FDA labels, how they move with each 2.5 mg step, and what they meant for the people who stayed on the drug. Nothing here is advice to start, stop or change a dose. It's what the trials recorded, laid out so you can have a sharper conversation with your prescriber.
Mounjaro side effects ranked by how often they showed up
The cleanest numbers come from the Zepbound label, which pools two weight-reduction studies with roughly 3,500 people. Stomach and gut complaints dominate the list. Everything else, from fatigue to dizziness, stays in single digits even at 15 mg.
| Side effect | Placebo | Zepbound 5-15 mg |
|---|---|---|
| Nausea | 8% | 25% to 29% |
| Diarrhea | 8% | 19% to 23% |
| Constipation | 5% | 11% to 17% |
| Vomiting | 2% | 8% to 13% |
| Abdominal pain | 5% | 9% to 10% |
| Injection site reaction | 2% | 6% to 8% |
| Fatigue | 3% | 5% to 7% |
Notice that the gap between drug and placebo is widest for nausea and vomiting. Some background noise is real, too: 8% of people on placebo reported nausea as well. You can read the full table in the Zepbound prescribing information.

Why the climb from 2.5 mg is when tirzepatide side effects peak
Both labels start everyone at 2.5 mg once a week and raise the dose in 2.5 mg steps, no sooner than every 4 weeks, up to a maximum of 15 mg. The Mounjaro label says this slow climb exists specifically to reduce gastrointestinal reactions. It also notes that the delay in stomach emptying is largest after the first dose and shrinks with time.
Each 2.5 mg step resets the clock a little. Many people describe the first few days after an increase feeling like week one again, which lines up with the label's note that GI events cluster during escalation.
What about side effects of Mounjaro 2.5 mg?
The label tables don't report a separate 2.5 mg arm, because 2.5 mg is a starting dose rather than a treatment dose. Every participant passed through it for 4 weeks on the way to 5, 10 or 15 mg. That's why side effects of Mounjaro 2.5 mg are mostly described in the same language as the escalation period as a whole: mild to moderate, and front-loaded.
- Week 1 to 4: 2.5 mg, the starting dose every trial used
- Week 5 onward: 5 mg, the lowest dose with full trial data
- Each later step: plus 2.5 mg after at least 4 weeks
- Ceiling: 15 mg once weekly for adults on both labels
If you want to see the whole ladder week by week, our tirzepatide dosage chart walks through each step as the labels describe it.

Mounjaro diarrhea, constipation and heartburn compared
Diarrhea climbs with the dose
Tirzepatide diarrhea followed a straight line upward in the weight-loss studies: 19% at 5 mg, 21% at 10 mg and 23% at 15 mg. In the diabetes trials behind the Mounjaro label, the same pattern held at lower overall rates, rising from 12% to 17%. Higher dose, more trips to the bathroom.
Constipation runs the other way
Mounjaro constipation is the odd one out. In the Zepbound data it was most common at 5 mg, at 17%, and dropped to 11% at 15 mg. We covered that quirk in detail in why Zepbound constipation peaks at 5 mg.
Tirzepatide heartburn and burping
Tirzepatide heartburn shows up in the label as two lines. Dyspepsia, or indigestion, reached 9% to 10% on the drug versus 4% on placebo, and reflux disease (GERD) reached 4% to 5% versus 2%. Burping was in the same range, at 4% to 5%.
| Measure | Mounjaro 5 mg | Mounjaro 15 mg |
|---|---|---|
| Nausea | 12% | 18% |
| Diarrhea | 12% | 17% |
| Decreased appetite | 5% | 11% |
| Vomiting | 5% | 9% |
| Any GI reaction | 37.1% | 43.6% |
| Stopped for GI reasons | 3.0% | 6.6% |
These Mounjaro figures come from about 950 adults with type 2 diabetes in the SURPASS-1 and SURPASS-5 trials. Placebo GI reactions ran at 20.4%, so roughly half of what people on 15 mg reported would likely have happened anyway.
What the side effects of tirzepatide cost in outcomes
Here's the number that matters most if you're weighing the trade-off: how many people quit because of side effects. In SURMOUNT-1, the 72-week trial of 2,539 adults with obesity, 4.3% stopped at 5 mg, 7.1% at 10 mg and 6.2% at 15 mg, compared with 2.6% on placebo. So roughly 93 to 96 people in 100 on tirzepatide did not stop for side effects.
What did staying on it buy them? Average body weight fell 15% at 5 mg, 19.5% at 10 mg and 20.9% at 15 mg, versus 3.1% on placebo, according to the American College of Cardiology trial summary. Put another way, the jump from 5 mg to 10 mg added 4.5 points of weight loss and about 3 extra quitters per 100. That's the kind of trade-off worth talking through with a clinician, not guessing at alone.
All-cause dropout in SURMOUNT-1 was actually higher on placebo, at 26.4%, than on any tirzepatide dose. Side effects pushed some people out, but lack of results pushed out more.

Mounjaro long term side effects and the cancer warning
The longest placebo-controlled data on the labels runs 72 weeks, so Mounjaro long term side effects beyond that window are still being studied. Both labels carry a boxed warning: in rats, tirzepatide caused thyroid C-cell tumors that depended on dose and duration. It's unknown whether that happens in humans, and the drug is contraindicated for anyone with a personal or family history of medullary thyroid carcinoma or MEN 2.
- Gallstones: 1.1% on Zepbound vs 1% on placebo
- Gallbladder inflammation: 0.7% on Zepbound vs 0.2% on placebo
- Acute pancreatitis: 0.2% in both drug and placebo groups
- Low blood sugar with a sulfonylurea: 10.3% vs 2.1% without
Those rarer events are why the labels list warning signs like severe stomach pain that doesn't let up. The numbers are small, but they're the reason prescribers ask about gallbladder and thyroid history before the first pen.
How to log side effects so your prescriber sees the pattern
A vague memory of feeling rough last month isn't much to work with. A dated log that ties each symptom to the dose you were on is. It turns a 10-minute appointment into a real discussion about whether the current step is working.
- Record the dose and injection day every single week
- Note symptoms with a start day and a rough 1 to 5 severity
- Mark each dose step so you can see what changed after it
- Track water and protein on days when appetite drops sharply
- Log injection sites to rule out local skin reactions
Redose was built for exactly this kind of record. It logs doses in one tap, rotates injection sites on a body map and keeps side effects next to the dose that preceded them. It's for tracking and record-keeping only; dose decisions stay with your prescriber. For vial-based protocols, the Redose dose calculators convert milligrams to syringe units.
Bring the log, not the memory. A pattern like nausea on days 2 and 3 after each injection is something a prescriber can act on.

Common questions about Zepbound side effects
Are Mounjaro side effects in women different?
The clearest difference on the Zepbound label is hair loss: 7.1% of women reported it compared with 0.5% of men. The label doesn't break out stomach side effects by sex. Our piece on GLP-1 hair loss covers why rapid weight loss tends to cause it.
Do tirzepatide long term side effects show up after a year?
In the trials, the pattern went the other way. GI complaints clustered in the escalation months and decreased over time, while the 72-week data didn't reveal a new wave of common effects later on. Longer outcome studies are ongoing, so treat anything past 72 weeks as unsettled.
Is there a link between Mounjaro side effects and cancer?
The only cancer signal on the label comes from rat studies of thyroid C-cell tumors, and it's unknown whether it applies to people. That's why the drug isn't prescribed to anyone with a personal or family history of medullary thyroid cancer or MEN 2. A new neck lump or trouble swallowing is worth raising with a doctor promptly.
What happens if I miss a weekly dose?
The Mounjaro label says to take a missed dose within 4 days (96 hours). If more than 4 days have passed, the label says to skip it and resume on the usual day. Your prescriber may give different instructions, and our missed dose guide covers the timing rules in more detail.
